update all forms in harvester to use same uswds form class

User Story

In order to provide a consistent UI, datagov wants to update all forms in the harvester app to use the uswds form class

Acceptance Criteria

[ACs should be clearly demoable/verifiable whenever possible. Try specifying them using BDD.]

• GIVEN we want/should use uswds as much as possible
  WHEN forms not using the uswds form class are updated
  THEN all of them will use the uswds form class \
