• ai4c-agent[bot] is authoring real PRs. PR #3692 (insect prothoracic tergum) and PR #3699 (guttural pouch) were both opened by ai4c-agent[bot], demonstrating the pipeline can independently produce and submit ontology term additions.
• Human domain experts are engaging. ANiknejad agreed with the proposed insect prothoracic tergum term on 2026-05-05, and cmungall is actively directing agent work on the guttural pouch PR. The collaboration model is working at the interaction level.

What seems not to be working

• Nothing is merging. With 0 merged PRs across the lookback window and several PRs explicitly flagged as ready by human commenters, the bottleneck is clearly at the final approval/merge step. Items are completing agentic workflows but then sitting idle.
• PR #3621 (sinus of lymph node, authored by dragon-ai-agent) has been stalled since KrishnaTO said "ready to merge" on 2026-01-19 — over three months of inaction. The corresponding issue #3586 also remains open despite the agent completing the work in November 2025.

Recurring failure modes and friction

• "Ready to merge" without a merger. Multiple items (PR #3621, PR #2593) have had human commenters explicitly state they are ready, but no maintainer with merge authority has acted. This suggests either a bottleneck on a specific maintainer (matentzn is @-mentioned as the gatekeeper in both cases) or a workflow gap where "approved" and "merged" are not connected.
