Support multiple signatures per SIGFILE

Currently each SafeTx signature must be in a separate file but some workflows may prefer all signatures to be in the same file.

In doing so, it may also be useful to support comments lines, so that signatures can be prefixed with some kind of description. To support this, any lines starting with a hash (`#`) can be ignored.
