[Moderator] “Add” button opens a blank blue screen

Description:
When clicking the “Add” button, the user sees a blank blue screen. No content loads and the action cannot be performed.

Preconditions
Log in as Модератенко (moderator@gmail.com).

Steps:

1. Go to "Адміністрування".
2. Go to "Заклади".
3. Press "Eye" image.
4. Press "Доповнити" button.

Actual result
A blank blue screen appears. No information or functionality is displayed.

The user should see the relevant information/form.
